Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared Dance Mafia during any occasion. In exchange for the acceptance of said child’s candidacy by  Dance Mafia. I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold Dance Mafia and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out any such activities.

I give my consent that if in an emergency medical situation arises, the organisation/club may act as a loco parentis. If the need arises for administration of first aid/or other medical practitioner may be necessary. I also understand that in such circumstances that all reasonable steps are made.

I understand that Dance Mafia is taking photographs/videos of my child not only for my child's development but to help promote the school.

I herby permit Dance Mafia to:

circulate the photographs to local newspapers and other parties

use the the photographs in any printed publications which may be produced

photographs/videos may appear online/social media

I understand that the fees are collected using a monthly subscription. Any discounted rates are worked out based on a fee for the term included any class holidays split by the number of months in the term with a payment break only in July.

Fees are collected at the start of a new month securing that space for the full month. If you wish to cancel your subscription you must do so before the 1st of the month or you will still be charged.
